An updated version of the Certificate of Installation form is now available
Are you completing the updated Certificate of Installation form when installing any gaming or monitoring equipment in your venue?
The Certificate of Installation — or COI form — has been updated to include sections for the installation of Ticket-In-Ticket-Out (TITO) and Card Based Cashless (CBC) equipment, and redesigned to be easier to fill out on any device and printed if necessary.
We have advised technical service providers, MAX and Aristocrat, to use the updated COI form instead of any previous versions.

Note there is still a requirement to forward the signed COI form to Intralot before switching on any installed gaming machines.
